Most are tubal, but interstitial, cervical, cesarean-scar, ovarian, abdominal, and heterotopic pregnancies require special management. The immediate clinical priority is to distinguish a stable patient who can undergo structured localization from a patient with **rupture and hemorrhage**, who needs resuscitation and urgent operative care.[1,2] In a stable patient with pain, bleeding, or an early pregnancy of uncertain location, obtain **transvaginal ultrasound (TVUS)** and quantitative serum **human chorionic gonadotropin (hCG)**, but do not use a single hCG threshold to decide pregnancy location. A **pregnancy of unknown location (PUL)** is a temporary state: no definite intrauterine or extrauterine pregnancy is visible despite a positive pregnancy test. Current NICE guidance places symptoms above hCG kinetics and recommends two hCG measurements as close as possible to **48 hours apart, but not earlier**.[1] For PUL, a rise in hCG **>63% over 48 hours** makes a developing intrauterine pregnancy more likely but does not exclude ectopic pregnancy; repeat TVUS is advised in 7–14 days, with earlier imaging considered when hCG is ≥1500 IU/L. A fall **>50%** makes ongoing pregnancy unlikely. A rise <63% or fall <50% warrants early clinical review because ectopic pregnancy remains possible.[1] Treatment of a confirmed unruptured tubal ectopic pregnancy is **expectant management, methotrexate, or surgery**, selected by symptoms, stability, hCG, mass size, embryonic cardiac activity, contraindications, fertility factors, and ability to return. NICE offers systemic methotrexate to a stable, pain-free patient with an unruptured tubal mass <35 mm, no visible cardiac activity, hCG <1500 IU/L, no intrauterine pregnancy, and reliable follow-up; it recommends surgery first when there is significant pain, mass ≥35 mm, visible cardiac activity, hCG ≥5000 IU/L, or inability to return.[1] These numeric cutoffs are jurisdiction-specific rather than universal biologic boundaries. A widely used single-dose methotrexate regimen is **50 mg/m² intramuscularly once**, with hCG measured on days 4 and 7. A **≥15% decline from day 4 to day 7** supports continued weekly monitoring; inadequate decline, rising hCG, or clinical deterioration requires reassessment for another dose or surgery.[2-4] Rupture can occur despite low or falling hCG, so symptoms always supersede a reassuring laboratory trend. > **Caution:** Never delay operative management in a patient with hemodynamic instability, peritoneal signs, progressive shock, or strong concern for rupture merely to obtain serial hCG values or perfect imaging confirmation. ## Classification | Site | Key feature | Main clinical issue | |---|---|---| | Ampullary tubal | Most common location | Typical tubal ectopic pathway | | Isthmic tubal | Narrower tubal segment | Earlier rupture can occur | | Fimbrial | Near distal tube | Tubal abortion or intraperitoneal bleeding | | Interstitial | Intramyometrial tubal segment | Can enlarge before rupture; hemorrhage may be severe | | Cervical | Implantation within cervix | High bleeding risk during treatment | | Cesarean-scar | Implantation in prior uterine scar niche | Hemorrhage, rupture, placenta accreta spectrum with continuation | | Ovarian | Implantation in ovary | Can mimic hemorrhagic corpus luteum | | Abdominal | Peritoneal implantation | Rare; high maternal morbidity | | Heterotopic | Coexisting intrauterine and ectopic pregnancies | Intrauterine pregnancy does not exclude ectopic disease | Most guideline thresholds below apply specifically to **tubal** ectopic pregnancy. Nontubal sites should be managed with specialist imaging and individualized medical, procedural, or surgical therapy. ## Pathophysiology and risk factors Normal fertilization occurs in the fallopian tube, followed by transport to the uterine cavity. Tubal inflammation, scarring, altered motility, or disturbed ciliary function can delay transport and permit extrauterine implantation. Trophoblastic invasion then erodes local tissue and vessels; the tube cannot accommodate unlimited growth, creating risk of rupture and hemoperitoneum. Risk is increased by: - Previous ectopic pregnancy. - Prior tubal surgery or sterilization failure. - Previous pelvic inflammatory disease, especially prior chlamydial infection. - Infertility and assisted reproduction. - Smoking. - Endometriosis or distorted tubal anatomy. - Increasing maternal age. - Pregnancy occurring with an intrauterine device in place: the absolute pregnancy risk is low, but a pregnancy that occurs is relatively more likely to be ectopic. Absence of a recognized risk factor does not meaningfully exclude ectopic pregnancy.[2] ## Clinical presentation Common symptoms are: - Unilateral or generalized pelvic/abdominal pain. - Vaginal bleeding or spotting. - Amenorrhea or a delayed menstrual period. - Dizziness, presyncope, or syncope. - Shoulder-tip pain from diaphragmatic irritation by blood. - Gastrointestinal or urinary symptoms that can mimic another acute abdominal disorder. ### Findings that suggest rupture or major bleeding | Finding | Concern | Action | |---|---|---| | Hypotension, tachycardia, poor perfusion, altered mentation | Hemorrhagic shock | Immediate resuscitation, blood preparation, urgent gynecology/operative activation | | Syncope or severe presyncope | Significant intraperitoneal blood loss | Treat as rupture until adequately excluded | | Generalized guarding or rebound | Hemoperitoneum/peritoneal irritation | Emergency surgical pathway | | Shoulder-tip pain with pelvic pain/bleeding | Diaphragmatic irritation from blood | Urgent reassessment | | Rapidly worsening focal pain | Expanding or rupturing ectopic | Immediate gynecology review | | Moderate/large complex free fluid on ultrasound | Hemoperitoneum | Escalate based on physiology and symptoms | Pregnancy physiology and young age can temporarily preserve blood pressure despite substantial blood loss. A single normal blood pressure should not override deteriorating symptoms, tachycardia, pallor, or poor perfusion. ## Initial assessment ### Immediate priorities 1. Assess airway, breathing, circulation, mental status, and perfusion. 2. Establish IV access if rupture is plausible. 3. Obtain CBC and blood group/RhD; add type and screen or crossmatch when major bleeding is possible. 4. Perform quantitative hCG and TVUS in stable patients. 5. Add renal and liver tests when methotrexate may be used. 6. Consider coagulation studies and blood gas/lactate in significant hemorrhage or shock. Pelvic findings may support the diagnosis, but no single physical finding safely rules ectopic pregnancy in or out. ## Ultrasound diagnosis TVUS is the preferred first-line imaging method for pregnancy location in a stable early-pregnancy patient.[1] ### Findings with high specificity A gestational sac separate from the ovary containing a yolk sac or embryo, with or without cardiac activity, is highly specific for ectopic pregnancy.[1] Other suspicious findings include: - An extraovarian tubal ring. - A complex or inhomogeneous adnexal mass moving separately from the ovary. - Hemoperitoneum or substantial pelvic free fluid. An empty uterine cavity, a nonspecific intrauterine fluid collection, or free fluid alone is not sufficiently specific to establish ectopic pregnancy. ### Heterotopic pregnancy A confirmed intrauterine pregnancy usually makes isolated ectopic pregnancy unlikely, but it does **not** exclude heterotopic pregnancy. Maintain suspicion when pain, free fluid, or an adnexal mass is disproportionate, particularly after assisted reproduction.[1] ## hCG: correct use and common misinterpretation ### Do not use hCG to declare location NICE explicitly advises against using serum hCG to determine where the pregnancy is located.[1] The historical concept of a fixed “discriminatory zone” is unsafe as a stand-alone rule because ultrasound quality, dating uncertainty, multiple gestation, and biologic variation can produce viable intrauterine pregnancies above commonly quoted thresholds before a gestational sac is seen. > **Clinical pearl:** hCG describes trophoblastic activity and trajectory; ultrasound establishes location. Neither should be interpreted without the patient’s symptoms. ## Pregnancy of unknown location PUL means that the pregnancy test is positive but TVUS has not shown a definite intrauterine or extrauterine pregnancy. Final outcomes include early viable intrauterine pregnancy, failing intrauterine pregnancy, ectopic pregnancy, or persistent PUL.[1] ### NICE serial-hCG pathway Obtain two serum hCG measurements as close as possible to **48 hours apart, but not earlier**.[1] | 48-hour hCG change | Interpretation | Next step | |---|---|---| | Rise >63% | Developing intrauterine pregnancy more likely; ectopic not excluded | Repeat TVUS in 7–14 days; consider earlier scan if hCG ≥1500 IU/L | | Fall >50% | Pregnancy unlikely to continue | Follow the recommended resolution pathway, including urine pregnancy testing at 14 days in the NICE pathway | | Rise <63% or fall <50% | Abnormal/indeterminate trajectory | Clinical review within 24 hours in an early-pregnancy service | | Any trajectory with worsening pain, syncope, heavy bleeding, or instability | Clinical deterioration overrides kinetics | Urgent reassessment for rupture or another acute cause | Do not give methotrexate simply because an early desired intrauterine pregnancy has not yet become visible. A viable intrauterine pregnancy must be adequately excluded before medical treatment for ectopic pregnancy.[1,2] ## Differential diagnosis | Diagnosis | Helpful clues | Key distinction | |---|---|---| | Threatened or early pregnancy loss | Vaginal bleeding, uterine cramping | Prior or current confirmation of intrauterine location changes the pathway | | Hemorrhagic corpus luteum | Ovarian cyst, pain, free fluid | Pregnancy location must still be established | | Ovarian torsion | Sudden severe pain, nausea, enlarged ovary | Urgent gynecologic imaging and surgical input | | Pelvic inflammatory disease | Fever, discharge, cervical tenderness | Infection testing and imaging; positive pregnancy requires location assessment | | Appendicitis | Focal right-sided pain, GI symptoms | Surgical pathway and pregnancy-adapted imaging | | Ureteric stone | Colicky flank-to-groin pain, hematuria | Urinalysis and renal imaging | | Heterotopic pregnancy | Intrauterine pregnancy plus persistent focal pain/adnexal lesion | Careful adnexal imaging, particularly after assisted reproduction | ## Treatment selection The decision among expectant management, methotrexate, and surgery is based on the whole clinical picture. | Factor | Favors conservative/medical pathway | Favors surgery | |---|---|---| | Hemodynamics | Stable | Instability or shock | | Pain | None or mild, nonprogressive | Significant or worsening pain | | Rupture | No evidence | Suspected or confirmed | | Mass | Small, unruptured | Large mass or concerning anatomy | | Embryonic cardiac activity | Absent | Present in many guideline pathways | | hCG | Lower and favorable trend | Higher values predict lower medical success; NICE uses ≥5000 IU/L for first-line surgery | | Follow-up | Reliable, rapid access to emergency care | Unable to return or unreliable access | | Methotrexate safety | No major contraindication | Contraindication present | | Fertility factors | May influence tube-preserving strategy | Hemorrhage control takes priority in instability | ## Expectant management Expectant care is appropriate only in a stable patient with minimal or no pain, reliable follow-up, no evidence of rupture, and a pregnancy likely to resolve without intervention. NICE recommends offering expectant management when all of the following apply:[1] - Tubal ectopic pregnancy <35 mm. - No visible cardiac activity. - Clinically stable and pain-free. - hCG ≤1000 IU/L. - Able to return for follow-up. It can be considered when hCG is >1000 and <1500 IU/L if the other criteria are met.[1] NICE follow-up uses hCG on days 2, 4, and 7 after the baseline assessment. If hCG falls by at least 15% at each interval, continue weekly testing until negative. If it plateaus, rises, or does not fall adequately, reassess for methotrexate or surgery.[1] ## Methotrexate Methotrexate inhibits folate-dependent DNA synthesis in trophoblastic tissue. It is appropriate only when the patient is stable, the ectopic is unruptured, a viable intrauterine pregnancy has been excluded, and reliable follow-up is possible.[1,2] ### NICE selection thresholds Offer methotrexate when all apply:[1] - No significant pain. - Unruptured tubal ectopic pregnancy. - Adnexal mass <35 mm. - No visible embryonic cardiac activity. - hCG <1500 IU/L. - No intrauterine pregnancy on ultrasound. - Reliable follow-up. For hCG **1500 to <5000 IU/L**, if the patient remains stable, has no significant pain, mass <35 mm, no visible cardiac activity, and can return, NICE recommends a choice of methotrexate or surgery after discussion of the higher likelihood of additional treatment.[1] ### Major reasons not to use methotrexate - Hemodynamic instability or suspected rupture. - Viable intrauterine pregnancy. - Inability to comply with serial hCG and emergency return. - Breastfeeding. - Significant hepatic dysfunction. - Significant renal dysfunction. - Clinically important anemia, leukopenia, or thrombocytopenia. - Active severe infection or major immunodeficiency. - Methotrexate hypersensitivity. Medication interactions, folate use, alcohol intake, and other patient-specific toxicities should be reviewed before treatment. ### Single-dose regimen A widely used regimen is:[2-4] - **Methotrexate 50 mg/m² IM once** on treatment day. - Measure hCG on **day 4 and day 7**. - If hCG falls by **≥15% from day 4 to day 7**, continue weekly hCG until the local negative/undetectable endpoint. - If the decline is **<15%**, hCG rises, or symptoms worsen, reassess urgently; options include repeat methotrexate under protocol or surgery. hCG may rise between treatment and day 4. This does not by itself indicate failure. The day-4-to-day-7 change is the key comparison in the single-dose protocol.[2-4] ### Monitoring after methotrexate Patients need explicit access instructions for new or worsening: - Severe abdominal or pelvic pain. - Shoulder-tip pain. - Syncope or presyncope. - Heavy bleeding. - Weakness, dyspnea, or collapse. Methotrexate does not immediately remove rupture risk. Continue surveillance until pregnancy tissue has resolved biochemically and clinically.[2] ## Surgical management ### Indications Surgery is required or strongly favored for: - Hemodynamic instability or rupture. - Significant persistent pain. - Major hemoperitoneum. - Methotrexate contraindication. - Failed medical or expectant treatment. - Inability to return for follow-up. NICE recommends first-line surgery for significant pain, ectopic mass **≥35 mm**, visible embryonic cardiac activity, hCG **≥5000 IU/L**, or inability to return.[1] ### Laparoscopy and laparotomy Laparoscopy is preferred when the patient can safely undergo minimally invasive surgery and expertise is available. Laparotomy may be needed with profound instability, uncontrolled hemorrhage, or technical circumstances requiring immediate open access.[1,2] ### Salpingectomy versus salpingotomy **Salpingectomy** removes the affected tube and is generally preferred when the contralateral tube appears healthy.[1] **Salpingotomy** removes the ectopic pregnancy while preserving the tube and may be considered when contralateral tubal disease makes fertility preservation particularly important. Persistent trophoblastic tissue is more common after salpingotomy, so serial hCG is mandatory.[1,5] The ESEP randomized trial found no significant improvement in subsequent ongoing pregnancy with salpingotomy compared with salpingectomy when the opposite tube was healthy, while persistent trophoblast occurred more often after salpingotomy.[5] NICE advises hCG 7 days after salpingotomy and then weekly until negative. After salpingectomy, a urine pregnancy test at 3 weeks is recommended in the NICE pathway, with reassessment if still positive.[1] ## Emergency management of suspected rupture ```text Positive pregnancy test + pain/bleeding ↓ Assess physiology immediately ↓ Instability / peritoneal signs / major free fluid / high concern for rupture? ├─ Yes │ → Large-bore IV access │ → CBC + coagulation studies + type/crossmatch │ → Blood products as indicated │ → Immediate gynecology and operating-room activation │ → Do not delay for serial hCG └─ No ↓ TVUS + quantitative hCG + CBC/RhD ↓ Definite IUP / definite ectopic / PUL ↓ If PUL: serial hCG + repeat TVUS, with symptoms overriding the laboratory trend If ectopic: choose expectant, methotrexate, or surgery based on risk criteria ``` ## Nontubal ectopic pregnancy ### Interstitial pregnancy An interstitial ectopic is located within the intramyometrial portion of the fallopian tube. Because surrounding myometrium can stretch, rupture may occur later and produce severe hemorrhage. Treatment may include systemic or local medical therapy, laparoscopic cornuostomy/wedge resection, or another specialist approach depending on size, cardiac activity, anatomy, and stability. ### Cervical pregnancy Cervical implantation can cause severe bleeding. Treatment may combine methotrexate, local injection, uterine-artery embolization, controlled suction evacuation, balloon tamponade, or surgery. Planning should occur in a center capable of hemorrhage control. ### Cesarean-scar pregnancy Early specialist diagnosis is essential because continued growth increases risk of uterine rupture, severe hemorrhage, and placenta accreta spectrum. Treatment is individualized and may include local or systemic medical therapy, ultrasound-guided procedures, hysteroscopic/laparoscopic resection, or combinations. ### Heterotopic pregnancy When a desired viable intrauterine pregnancy coexists, systemic methotrexate is usually unsuitable because it would harm the intrauterine pregnancy. Management of the ectopic component is usually surgical or uses a targeted local intervention.[2] ## RhD prophylaxis: UK 2026 update NICE changed UK recommendations on **17 June 2026**:[1] - For ectopic pregnancy or miscarriage **up to and including 11+6 weeks**, do **not** offer anti-D immunoglobulin prophylaxis. - At **12+0 to 12+6 weeks**, offer at least **250 IU (50 micrograms)** to RhD-negative patients undergoing medical or surgical management. > **Current practice:** Anti-D guidance differs by jurisdiction. The UK thresholds above should not be applied automatically in a country whose transfusion or obstetric guidance differs. ## Future fertility and recurrence Future fertility depends mainly on baseline tubal health, contralateral-tube status, maternal age, prior pelvic infection, infertility history, and treatment type. A previous ectopic pregnancy increases recurrence risk. In a subsequent pregnancy, early contact with obstetric care and early ultrasound to confirm location are appropriate.[2] ## Psychological support Ectopic pregnancy is both a potentially life-threatening event and a pregnancy loss. Clear communication should include: - The current degree of diagnostic certainty. - The reason serial testing may be needed. - Exact emergency return symptoms. - Expected duration of hCG follow-up. - Fertility implications and recurrence risk. - Access to pregnancy-loss and mental-health support when desired.[1] ## Practical checklist ### Unstable or suspected rupture - [ ] Resuscitate while activating gynecology/operative care. - [ ] Large-bore IV access. - [ ] CBC, coagulation studies, type/crossmatch. - [ ] Blood products as indicated. - [ ] Do not wait for serial hCG. ### Stable patient - [ ] Quantitative hCG. - [ ] TVUS. - [ ] CBC and RhD status. - [ ] Assess pain, bleeding, syncope, and free fluid. - [ ] Classify as intrauterine pregnancy, definite ectopic, or PUL. - [ ] If PUL, arrange 48-hour hCG pathway and repeat imaging. - [ ] If ectopic, document expectant/medical/surgical eligibility and follow-up reliability. ### Before methotrexate - [ ] Viable intrauterine pregnancy excluded. - [ ] No evidence of rupture. - [ ] CBC, renal function, liver tests reviewed. - [ ] Contraindications and interacting medicines reviewed. - [ ] Day 4/day 7 hCG already arranged. - [ ] Emergency return instructions provided. ## References 1.
